So what are his 'breaths' controlling, I'm guessing MiDi pitch, perhaps velocity as well , and his left hand ?

There is a specified controller called "breath controller" in MiDi specification.
Possibly he used that one?

You can purchase a breath controller and retrofit it to a wind instrument.

They did all of the heavy lifting for you, in that they calibrated the pressure sensor and provided an interface.
