I need a circuit that can take a 0-5V input and convert it into a PWM output from 0-100%. Would prefer if the output was 5V logic level.

How linear does it have to be?

If it's just to control a lamp or motor I'd just use a dual rail-to-rail comparator such as the MC6772.
