Why is Common Mode Voltage so Commonly Misunderstood?

It is important to understand these are Abs. Max for inputs and that these all 3 conditions must be met, not conflicting with the others, single input wrt to gnd. , paired inputs... DM, CM , in case of crowbar protection added. n.b.,
These are not normal operating ranges.
