What can be the reason behind this tragedy?

In my country, a mother died along with her daughter, whom she was holding in her arms while cooking rice in a rice cooker. She plugged it in and got electrocuted. What can be the reason behind this electrocution? Where might the fault be?
 
You're Bangladesh?

You can start with lax inforcement of people stealing power with illegal splices into the grid. If the taps into the grid are not authorized, there is likely no inspection, no circuit breakers, no grounding, no GFCI, ...

It's sad but you get what you enforce (or what you don't enforce).

Good old USA has done Bangladesh like things in the past. Like the
"all American 5" tube radios with hot chassis. Dont be station selecting
or changing volume while taking a bath.....

If its stupid we done it here, and rinse and repeat in many instances.
Like industrial pollution. Been doing that in my small town, had to raise
$5M to filter water supply from known cancer causing chemicals. And
progenitor who did that got kicked out of another State and we welcomed
them in knowing that.

Stupid is as stupid does.....
 
Unfortunately, many safety regulations are only legislated after an accident has already happened.

Hopefully this tragedy will prompt legislators into action. That would be the redeeming quality for the accident.
