I have a frequeny input of 4 volts from a sensor but the PLC I wish to use needs min 15 volts. Can anyone provide me with a simple solution? Perhaps a schematic?

You mean output right?

What kind of frequency? Square wave? Sine wave? What frequency range? Unipolar? Bipolar? (probably unipolar right?)

Either way, you could probably just use an op-amp amplifier with a gain of 15/4 = 3.75. THe op-amp would need to be able to output 15V so it would need a power source of 15V (if it's capable rail-to-rail output) or higher. You can google for a schematic...you want a "non-inverting amplifier op-amp" circuit. They are everywhere.
