Ubuntu LibreOffice Glitch with Dock

    Every once in a while, (very rare), when I right click on the Dock for LibreOffice Writer, the middle selection changes.

    It's supposed to be:
    New Document
    LibreOffice Writer (this one changes)
    Unlock from Launcher

    System is UBUNTU 12.04 LTS 64 bit running from CD
    LibreOffice Build ID: 350m1 Build 2 (supplied with CD)

    The last time, it happened, the selection went blank, hence I could not laucch LibreOffice.
    Other times, it has been replaced by other programs such as Bittorrent.

    Since nothing is persistent, I can re-boot and all is well, but I'd like to know why and what is changing and how to bring it back without re-booting.

    Any ideas?
