Little bit doubt on the connections. Is my drawing ok?

Yes, but you should only need one transformer, at the DVR end. It's just to break the "ground loop" that allows the audio cable to also be part of the power circuit.

Try it either way around, (33 ohm to the mic or to the DVR) and see which gives best audio.
