Smart Voltage Protection for PC

Im working on a project that should protect my server from a "death" because of too high voltages.

The whole thing should be plugged into a molex plug and continuously check the 5v and 12v channel. If the voltages should get too high or too low it should make the two power-on pins on the mainboard connect. This way the computer will shut down by itself.

Id appreciate any ideas on how to do this project.

I mean - if the power supply healthy - never give overvoltage,because contains protections. So when overvoltage appear, the power supply soft-shutdown not help. Connect serial with each outputs a fuse, and after fuse a power zener.
(most of case in ATXPSU-s blow out the standby section: give overvoltage to controller chip, and all outputs go extremly high: 5V to 8V, 12V to16V!)
