Identify Transformer

I need help identifying a transformer.



The transformer has the following written on one side:

LS-A12119-PT
E154515 Z150H
CLASS B (130 C)
LEI-4 0704


I have attached a picture.
Pins 1 and 4 are the inputs (pin 4 = Ground)
Pins 5 - 8 are outputs, with 6 and 7 being common.
The output is 12V.

Anyone have any leads on this?

It looks like a small mains transformer.

If in doubt connect it to the mains via a small light bulb, I'd suggest 15W, normally 60W-100W is recommended but this will probably blow such a small transformer.
