help, voltage buffer issue

I have a voltage buffer (OPA4354AIDR) which has a 2mV decrease in voltage bias.
However, it drops more like 5 mV and one of the 4 channels on the buffer drops about 60 mV which I cant accept. Any idea how to fix this?

The impedance from all outputs and inputs to ground is very similar.

i discovered it was another circuit that this one attaches too that had a blown IC.... 6 hours later
