Need help with wiring

I want to to make up a simple box that takes a mono audio input to a switch that will allow me to go Left, OR Right Or center with the output signal and I would need to have a volume control in the circuit.

What switch and what potentiometer would I need, and how would I wire it up?

I would really appreciate any help.
