Need help identifying this connector

Would really appreciate if anyone could help me out here, as you can see one of them shorted out and melted itself

I'm not understanding what you're asking, are you wanting to replace the connector or help identifying the shorted leads?
 
I have a few loose SMT connectors from recycled PCBs similar to this, and matching cables. Send me the dimensions of your connectors and I'll check my stock. I'm in Alberta, Canada.
