This is something I finished a while back with some valuable help from Tom. I tend to be distracted when doing stuff and made loads of silly errors... thanks Tom!
I've currently sent this schem for production as a SMT PCB that is half the width of a credit card. Will be getting the PCBs sometime next week. Here is an image of the PCB layout:
