I need opamp suggestions

Glad that you were able to figure it out....
Having said this, I also find extremely annoying that the electronics industry is starting to move away from common opamp pinouts.

I was aware of a 5-pin single opamp, but wasn't aware of the different pinout arrangements.

In the old days, only with quad packages one had to be careful.
Only the RC4136 had an oddball pinout. If I remember properly, when TI brought their highly successful line of TL0x4 opamps, they actually made a RC4136-compatible version.
