Default Single ended to differential converter

Hi

I need to convert a single ended 5 volt signal (from a pc parallel port) to a 12 volt differential signal that goes to a double ended opto-coupler. Max pulse rate .5 MHz

I tried it out with a differential output shaft encoder-works great but now I need to hook it up to the 'puter.
