Source for artist's lamp switch? - Electronic Circuits Projects Diagrams Free

Wife would like her lamp fixed! Switch's life appears to have run its course. One push button for incandescent, one for fluoro and off button in the center.

I haven't been able to find anything even similar in Allied and Mouser catalogs, nor on Google searches.

Any ideas where to look for something that would work? Doesn't necessarily have to be identical.
